Registration for Students Entering Grades 6 through 8

We are very excited to welcome new families to Stafford Middle School!

Families who wish to enroll a child in grades 6 - 8, should submit an enrollment packet along with the required documentation. For your convenience, enrollment forms are available below for downloading. If you need further assistance, please contact Stafford Middle School at (860) 684-2785.

After completing the enrollment form you will have the opportunity to upload supporting documentation for registration and data collection purposes, prior to submittal.

Step 1. PowerSchool Registration

Complete PowerSchool Online Registration (Follow the link.)

Registration Documents:

• Long Form Birth Certificate
• Proof of Residency or Residency Affidavit Form

Data Collection Documents:

• Transfer of Confidential Student Information (This form is used to transfer your child's educational records from their previous school.)
• Current Health Assessment AND Proof of Immunizations
• Parent / Guardian Photo ID
• Custody Alert with Court Order, if applicable
• Current Report Card
• Disciplinary and Attendance Reports from Previous School
• Copy of Current IEP or 504 Plan, if applicable

Step 2. Schedule an Appointment

Once you have submitted the enrollment form and uploaded the documentation, schedule an appointment by contacting the school office at (860) 684-2785.
